PDT 치료는 원래 여드름 치료 목적으로 시행하는데
여드름을 좋아지게 하는 효과 중의 하나가 피지 분비를 줄이는 것입니다.

여드름 치료에 사용하는 광선치료(photodynamic therapy, PDT)는
기본적으로 광선의 효과를 높여주는 약물을 바른 후에
광선을 쬐서 여러 가지 치료 효과를 나타냅니다.
이 때 사용하는 광선을 어떤 종류를 사용하느냐에 따라
치료 효과나 부작용 등이 달라지게 됩니다.
여드름 치료에 효과적인 이유는
광선의 효과를 높여주는 약물이 피지를 만들어내는 피지선에
선택적으로 더 많이 흡수되어
광선을 쬐었을 때 더 많이 파괴되기 때문입니다.
따라서 피지 분비가 줄어들게 되지요.
또한 여드름의 염증을 일으키는데 관여하는 여드름 균을 박멸하는 효과도 나타냅니다.
그래서 염증성 여드름의 치료에 더 효과적입니다.
PDT 치료에 사용할 수 있는 광원(light source)에는 여러 가지가 있습니다.
IPL, PPx, 브이빔이나 브이스타 같은 색소 레이저, 블루 라이트 등입니다.
다 나름대로 효과가 있습니다.
어떤 광원이 가장 좋은 지에 대해서는 이견이 있는 상태인데
기본적으로 치료 목적에 따라 광원을 선택하는 것이 좋다고 되어 있습니다.
즉, 여드름 치료를 위해서라면 여드름 치료에 원해 효과적인 광원을 선택하고
전반적인 피부 톤을 밝게 하고 잡티나 혈관을 줄이기 위해서는
원래 이런 쪽에 효과가 좋은 IPL을 선택하는 것이 좋다는 것입니다.
PPX는 그 자체로도 여드름 치료에 효과적입니다.
진공을 이용해서 모낭 내에 들어있는 피지를 뽑아내고
400나노미터 대의 파장의 빛이 여드름 균을 줄여주기 때문에
염증성 여드름 치료에 효과적일 뿐 아니라
피지선을 위축시키는 효과가 있어서입니다.
여기에 광선의 효과를 더해주는 약물을 더해서 치료하면(즉, PDT 치료를 하면)
그 효과가 더 증폭됩니다.
그래서 저의 병원에서는 여드름 치료를 위해 PDT 치료를 할 때
주로 PPx를 광원으로 이용하고 있습니다.
경우에 따라 IPL 을 광원으로 이용하기도 하고요.
이런 PDT 치료를 3회 정도 반복하면
6-12개월 정도 여드름이 현저히 줄어들게 됩니다.
하지만 여드름이 전혀 안생긴다는 것은 아니며
간간이 생길 수 있습니다.
피지 분비도 마찬가지여서 치료 후에 줄기는 하지만
완전히 없어지는 것은 아닙니다.
PDT 치료에 사용하는 광선과민제는 피부에서 없어지는데 40시간 정도 걸립니다.
따라서 PDT 치료 후에는 약 40시간(이틀) 정도 햇볕을 피해야 합니다.
그대로 햇볕에 노출되면
여름에 해수욕장에서 심하게 햇볕에 노출되었을 때와 비슷한 상태가 될 수 있습니다.
최근에 mild PDT라고 해서 햇볕을 피할 필요 없이
